Below is a list of documents you must provide to the HR Manager with Metropolitan Mental Health Clinic, Inc. (MMHC) before we can offer you a position with our agency. Once these documents have been received and reviewed and we are interested in employing you - the HR Manager will call you to set up an interview.

1. (MMHC) Application
2. Resume
3. Education Degree or Copy of Transcript
4. Social Security Card/Birth Certificate or Green Card
5. State Driver's License
6. Drivers Record/Report from Dept of Motor Vehicle in the State where you are licensed
7. Proof of Automobile Insurance
8. Proof of Liability Insurance
9. OMHC Clinical Privileges Form

We also do a criminal history background check on our employees which will require that you be fingerprinted. Your prints will be forwarded to CJIS - and they in turn will initiate your background investigation, which is a cost to you, the total will be $52.00. ($10.00 for your prints and $42.00 for CJIS to initiate your background investigation).

We will withdraw an employment offer if the investigation reveals information that precludes a security/or unsuitability clearance (e.g. serious abuse of alcohol, DUI, assault or child abuse, misrepresentations during the application process, etc.)

Along with the above you will also be required to have the following mandatory training:

1. CPR Certification
2. First Aid Certification
3. Infectious Disease Training
4. HIPAA (Health Insurance Portability & Accountability Act

The HR Manager will give you a list of Red Cross Agencies you can call to set up an appointment for your CPR/Safety training. You have 30 days after your date of hire in which to complete this training.

You will receive Infectious Disease and HIPAA training at orientation. You will also be filling out additional papers at that time.
